Complex medical conditions require teamwork. Working as a team with patients and their families, our staff develops individualized treatment plans and sets goals which meet the needs of everyone involved. Together, the team determines the best ways to help patients maximize their strengths, enhance areas in which they need improvement and become as independent as possible.

CARE

Coordinated Treatment Center

The Sanford Coordinated Treatment Center is located within Sanford Broadway Health Center. The programs offered by Sanford Hospital and Sanford Children's Hospital have been designed specifically for disabled adults and children. The Coordinated Treatment Center assists primary care doctors in the management of their patients' complex health problems. Program evaluations are always available to the service agencies patients work with.
